Wegovy 0.25mg Flextouch Pre Filled Pen contains semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist. It is used as an adjunct to a reduced-calorie diet and increased physical activity for weight management in adults and adolescents, and to reduce the risk of major adverse cardiovascular events in adults with established cardiovascular disease. It also holds provisional approval for the treatment of Metabolic Dysfunction Associated Steatohepatitis (MASH) in adults.

Uses and Indications

  • Weight management (weight loss and maintenance) in adults with a Body Mass Index (BMI) of ≥30 kg/m² (obesity) or a BMI of ≥27 kg/m² to <30 kg/m² (overweight) in the presence of at least one weight-related comorbidity (e.g., dysglycaemia, hypertension, dyslipidaemia, obstructive sleep apnoea, or cardiovascular disease).
  • Weight management in adolescents aged 12 years and above with obesity (BMI at or above the 95th percentile for their age and gender) and weighing more than 60 kg.
  • Reduction of major adverse cardiovascular events (cardiovascular death, non-fatal myocardial infarction, or non-fatal stroke) in adults with established cardiovascular disease and either obesity or overweight.
  • Treatment of Metabolic Dysfunction Associated Steatohepatitis (MASH) in adults with moderate to advanced liver scarring (fibrosis stages F2–F3) without cirrhosis.

The Wegovy 0.25mg Flextouch Pre Filled Pen contains:

  • Active Ingredient: Semaglutide 0.25mg.
  • Excipients: Disodium phosphate dihydrate, sodium chloride, water for injection. Phenol and propylene glycol are also inactive ingredients in the FlexTouch pen. Hydrochloric acid or sodium hydroxide may be added to adjust pH.

Dosage and Administration

  1. Wegovy 0.25mg is the recommended starting dose, administered subcutaneously once weekly.
  2. The dose is gradually increased every 4 weeks under a doctor's guidance to reduce gastrointestinal side effects, until a maintenance dose is reached.
  3. Inject subcutaneously in the abdomen, thigh, or upper arm. Rotate injection sites each week.
  4. Administer once weekly on the same day each week, at any time of day, with or without meals.
  5. Visually inspect the solution before use; it should be clear and colorless. Do not use if cloudy, colored, or contains particles.
  6. Each pre-filled pen contains 4 doses. Do not share your pen with other people.

 

Storage and Disposal

  • Store unopened pens in the refrigerator between 2°C to 8°C (36°F to 46°F) in the original carton to protect from light.
  • Do not freeze. Discard the pen if it has been frozen.
  • An unopened pen can be stored at room temperature (below 30°C / 86°F) for up to 28 days (US) or 6 weeks (outside US) if the cap has not been removed.
  • Once opened, a FlexTouch pen can be stored in the refrigerator (2°C to 8°C) or at room temperature (below 30°C) for up to 6 weeks.
  • Dispose of used pens and needles in an FDA-approved sharps container according to local requirements.

Precautions and Safety

  • Wegovy is not a substitute for insulin.
  • There is a risk of hypoglycemia, especially when used with insulin or sulfonylureas. Blood sugar levels may need monitoring, and doses of other diabetes medications may require adjustment.
  • Acute pancreatitis has been observed. Seek medical attention immediately for severe, persistent stomach pain.
  • Gallbladder problems, including gallstones, can occur. Consult a doctor for symptoms such as severe stomach pain, nausea, vomiting, or yellowing of the skin or eyes.
  • Dehydration may result from gastrointestinal side effects like nausea, vomiting, and diarrhea. Ensure adequate fluid intake, particularly if kidney problems are present.
  • Serious allergic reactions (e.g., anaphylaxis, angioedema) are possible. Discontinue use and seek immediate medical help for symptoms such as rash, shortness of breath, swelling of the face, lips, or tongue, or a fast pulse.
  • Not recommended during pregnancy or for at least two months before a planned pregnancy; effective contraception should be used.
  • Not recommended for breastfeeding mothers.
  • There is a risk of thyroid C-cell tumors, including medullary thyroid carcinoma (MTC). Wegovy is contraindicated in patients with a personal or family history of MTC or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • Diabetic eye disease (retinopathy) may worsen. Monitor for any vision changes.
  • May affect the absorption of other oral medicines due to slowed gastric emptying.
  • Increased heart rate has been reported.
  • Self-administration is not recommended for visually impaired individuals.

 

Side Effects and Risks

  • Very common (affecting more than 1 in 10 people): Headache, nausea, vomiting, diarrhea, constipation, stomach pain, feeling weak or tired.
  • Common (affecting up to 1 in 10 people): Dizziness, upset stomach or indigestion, burping, gas (flatulence), bloating, inflamed stomach (gastritis), reflux or heartburn, dry mouth, gallstones, hair loss, injection site reactions, altered taste sensation, changes in skin sensation, low blood sugar (in patients with diabetes).
  • Uncommon (affecting up to 1 in 100 people): Low blood pressure, fast heartbeat, delayed stomach emptying, increase in pancreatic enzymes.
