Dosage and Administration

  1. The recommended starting dose is 2.5 mg injected subcutaneously once weekly for 4 weeks.
  2. After 4 weeks, the dose is typically increased to 5 mg once weekly.
  3. Further dose increases may be made in 2.5 mg increments after at least 4 weeks on the current dose, as determined by your doctor.
  4. The maximum recommended dose for adults is 15 mg once weekly.
  5. Administer the injection subcutaneously in the abdomen, thigh, or the back of the upper arm.
  6. Mounjaro KwikPen is a multi-dose pen containing 4 fixed doses; discard the pen after 4 doses.

 

Storage and Disposal

  • Store unused Mounjaro KwikPen in a refrigerator between 2°C and 8°C. Do not freeze.
  • Protect the medicine from light by storing it in its original carton.
  • An unopened pen may be stored unrefrigerated for up to 30 days at a temperature not above 30°C, after which it must be discarded.
  • After first use, the pen can be stored at room temperature (up to 30°C) or in the refrigerator (2°C to 8°C) for up to 30 days. Discard the pen after 30 days or after 4 weekly doses, whichever comes first.
  • Keep out of reach of children.
  • Dispose of used pens and needles in a sharps disposal container according to local guidelines.

Precautions and Safety

  • Mounjaro carries a Boxed Warning regarding the risk of thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), observed in rats. It is unknown if this risk applies to humans.
  • Do not use Mounjaro if you or your family have a personal or family history of MTC or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • Inform your doctor if you have a history of pancreatitis, as acute pancreatitis has been reported with tirzepatide.
  • Be aware of symptoms of low blood sugar (hypoglycemia), especially if using Mounjaro with sulfonylurea or insulin.
  • Avoid driving or operating machinery if you experience symptoms of low blood sugar.
  • Tell your healthcare provider about all medicines you take and any upcoming surgeries or procedures.
  • Serious allergic reactions, including anaphylaxis and angioedema, have been reported.
  • Gastrointestinal adverse reactions may lead to dehydration, which could cause acute kidney injury.

 

Side Effects and Risks

  • Common side effects include nausea, diarrhea, decreased appetite, vomiting, constipation, indigestion, and stomach (abdominal) pain. These are often mild to moderate and may decrease over time.
  • Serious side effects can include inflammation of the pancreas (pancreatitis), severe low blood sugar (hypoglycemia), serious allergic reactions, dehydration leading to kidney problems, severe stomach problems, changes in vision, and gallbladder problems.
  • Contact your doctor immediately if you experience severe or persistent side effects, unceasing vomiting, severe stomach pain, trouble breathing, swelling of the face or throat, signs of dehydration, yellowing of the skin or eyes, confusion, fainting, or vision changes.
