Romy 500mg Injection

Romy 500mg Injection contains Romiplostim 500mcg, a thrombopoietin receptor agonist used to treat chronic immune thrombocytopenia (ITP) in patients who have not responded adequately to other treatments.

Benefits

Romiplostim works by stimulating the bone marrow to produce more platelets, which helps to increase platelet counts. This action reduces the risk of bleeding events and the need for other treatments in patients with low platelet counts. It is effective in improving platelet counts in various settings, including chronic ITP.

Uses and Indications

• Treatment of thrombocytopenia in adult patients with chronic immune thrombocytopenia (ITP) who have had an insufficient response to other treatments like corticosteroids, immunoglobulins, or splenectomy.
• To increase survival in adults and pediatric patients acutely exposed to myelosuppressive doses of radiation (Hematopoietic Syndrome of Acute Radiation Syndrome - HS-ARS).

The Romy 500mg Injection contains:

• Romiplostim 500mcg (active ingredient).
• Excipients: Mannitol, Sucrose, L-Histidine, Polysorbate 20, and Hydrochloric Acid (for pH adjustment).

Dosage and Administration

1. Romy 500mg Injection is administered by a doctor or nurse as a weekly subcutaneous injection.
2. The initial dose is typically 1 mcg/kg of body weight, adjusted weekly based on the patient's platelet count response to maintain a platelet count of ≥ 50 x 10⁹/L. The maximum weekly dose should not exceed 10 mcg/kg.
3. The powder in the vial is reconstituted with sterile water for injection. Gently swirl and invert the vial to dissolve the contents; do not shake or vigorously agitate.
4. A syringe with 0.01 mL graduations should be used for precise administration, as injection volumes can be very small.

Storage and Disposal

• Store unopened vials in a refrigerator between 2°C to 8°C. Do not freeze.
• Protect from light by storing in the original carton.
• Unopened vials may be stored at room temperature (up to 25°C) for a single period of up to 30 days. Once stored at room temperature, do not return to the refrigerator.
• After reconstitution, the solution can be kept at room temperature (25°C) or refrigerated (2°C to 8°C) for up to 24 hours, protected from light.
• This is a single-use vial; any unused portion must be discarded.
• Keep out of reach of children.

Precautions and Safety

• Romy 500mg Injection is not indicated for the treatment of thrombocytopenia due to myelodysplastic syndromes (MDS) or any cause other than chronic ITP. Use in MDS patients may worsen the condition.
• There is an increased risk of blood clots if platelet counts become excessively high during treatment. Your doctor will monitor your platelet levels closely.
• This medicine may cause changes in the bone marrow, including increased reticulin and possible bone marrow fibrosis.
• Inform your doctor about any history of blood clots, bleeding problems, liver disease, or kidney problems before starting treatment.
• Avoid activities that may cause injury or bleeding during treatment.
• Do not breastfeed during treatment with romiplostim.

Side Effects and Risks

• Common side effects include headache, joint pain, insomnia, muscle pain, pain in extremity, abdominal pain, indigestion, paresthesia (tingling or pricking sensation), dizziness, and injection site reactions (redness, swelling, bruising).
• Serious side effects, though rare, can include allergic reactions (skin rash, itching, hives, swelling of face/lips/tongue/throat), and blood clots (pain/swelling/warmth in leg, shortness of breath, chest pain, sudden dizziness).
• Worsening of a precancerous blood condition (MDS) to acute leukemia has been reported.
