Uses and Indications

  • As an adjunct to diet and exercise for the treatment of type 2 diabetes mellitus in adults.
  • For chronic weight management in adults with obesity (BMI ≥ 30) or overweight (BMI ≥ 27) with at least one weight-related comorbidity, as an adjunct to diet and exercise.
  • Treatment of moderate to severe obstructive sleep apnea (OSA) in adults with obesity.

The Mounjaro 12.5Mg Kwikpen contains:

  • Tirzepatide 12.5 mg (active ingredient).
  • Excipients include sodium chloride, sodium phosphate dibasic heptahydrate, and water for injection.
  • Each 0.6 mL dose contains 5.4 mg benzyl alcohol.
  • Hydrochloric acid solution and/or sodium hydroxide solution may be added to adjust pH.

Dosage and Administration

  1. Mounjaro 12.5Mg Kwikpen is administered as a once-weekly subcutaneous injection.
  2. Rotate the injection site with each dose, using the abdomen, thigh, or upper arm.
  3. Before use, visually inspect the solution; it should be clear and colourless to slightly yellow. Do not use if particulate matter or discoloration is observed.
  4. Do not share your Mounjaro Kwikpen with other people, even if the pen needle has been changed.
  5. Each Kwikpen contains four fixed doses. Discard the pen after administering the fourth dose.

 

Storage and Disposal

  • Store unused Mounjaro Kwikpen in the refrigerator at 2°C to 8°C (36°F to 46°F) in its original carton to protect from light.
  • Do not freeze Mounjaro. Do not use if it has been frozen.
  • Once removed from the refrigerator, the Kwikpen can be stored at room temperature (up to 30°C or 86°F) for up to 30 days.
  • Dispose of used Kwikpens and needles in a sharps disposal container.

Precautions and Safety

  • Tirzepatide has caused thyroid C-cell tumors in rats; it is unknown if this occurs in humans. Mounjaro is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • Acute pancreatitis has been reported with tirzepatide. Observe patients for symptoms such as persistent severe abdominal pain.
  • Concomitant use with insulin secretagogues or insulin may increase the risk of hypoglycemia. Dose reduction of these concomitant medications may be necessary.
  • Serious hypersensitivity reactions, including anaphylaxis and angioedema, have been reported.
  • Gastrointestinal adverse reactions like nausea, vomiting, and diarrhea can lead to dehydration and acute kidney injury; monitor renal function.
  • Use with caution in patients with a history of diabetic retinopathy or acute gallbladder disease.
  • Mounjaro may affect the efficacy of oral hormonal contraceptives; consider switching to a non-oral method or using a barrier method for 4 weeks after initiation and each dose escalation.
  • Avoid alcohol consumption as it may mask or worsen symptoms of hypoglycemia.
  • This medicine is not indicated for patients with type 1 diabetes.

 

Side Effects and Risks

  • Very common side effects include nausea, diarrhea, vomiting, and constipation.
  • Common side effects include decreased appetite, indigestion, abdominal pain, belching, gas, heartburn, fatigue, injection site reactions (redness, swelling, itching), dizziness, low blood sugar, increased heart rate, gallstones, and hair loss.
  • Serious side effects can include acute pancreatitis, severe allergic reactions, acute kidney injury, and acute gallbladder disease.
