Topme Injection 5ml contains Metoprolol Tartrate 1mg/ml, a beta-blocker primarily used to manage various cardiovascular conditions by affecting the heart and blood circulation.

Uses and Indications

• Relief of chest pain (angina pectoris).
• Control of irregular heart rhythms (arrhythmia).

The Topme Injection 5ml contains:

• Metoprolol Tartrate IP 1mg/ml (active ingredient).
• Sodium Chloride USP.
• Water for Injection USP.

Dosage and Administration

1. Topme Injection 5ml is for intravenous administration only and should be given by a registered competent doctor or nurse/midwife.
2. For acute myocardial infarction, treatment typically begins with three bolus injections of 5 mg (5ml) of Metoprolol Tartrate each, administered at approximately 2-minute intervals.
3. Blood pressure, heart rate, and electrocardiogram should be closely monitored during intravenous administration.
4. Following intravenous therapy, patients are usually transitioned to an oral formulation of metoprolol.

Storage and Disposal

• Store at 20°C to 25°C (68°F to 77°F), maintaining USP Controlled Room Temperature.
• Protect from light and heat; retain in the original carton until the time of use.
• Do not freeze the injection.
• Discard any unused portion of the vial.

Precautions and Safety

• Do not stop Metoprolol Tartrate suddenly, as this may worsen your condition and increase the risk of adverse cardiac events.
• Exercise caution in patients with a history of heart failure, bronchospastic disease, or pheochromocytoma.
• Metoprolol may mask the symptoms of low blood sugar (hypoglycemia), particularly in diabetic patients. Monitor blood glucose levels regularly.
• Monitor heart rate and blood pressure closely during treatment.
• Avoid operating machinery or driving until you know how the medication affects you, as it may cause dizziness or fatigue.

Side Effects and Risks

• Common side effects include dizziness, fatigue, headache, nausea, stomach pain, and breathlessness.
• Serious side effects to report immediately include very slow heartbeats, signs of heart failure (shortness of breath, swelling), severe allergic reactions (rash, hives, swelling of face/throat), or coldness in hands and feet.
• Orthostatic hypotension (sudden drop in blood pressure upon standing) may occur.
