Sodinate 8.4% 25ml Injection contains Sodium Bicarbonate 8.4% w/v, an electrolyte replenisher and systemic alkalizer used to treat metabolic acidosis and other conditions requiring systemic alkalinization.

 

Benefits

Uses and Indications

  • Metabolic acidosis, which can arise from conditions such as severe renal disease, uncontrolled diabetes, severe dehydration, cardiac arrest, or shock.
  • Severe diarrhea accompanied by a significant loss of bicarbonate.
  • Certain intoxications or poisoning, including those by salicylates, methanol, phenobarbitone, or lithium, to decrease renal absorption of the drug or correct associated acidosis.
  • Increasing urinary pH to enhance the solubility and excretion of certain weak acids like cystine, sulphonamides, and uric acid.

Dosage and Administration

  1. Sodinate 8.4% 25ml Injection is administered intravenously.
  2. The precise dosage is determined by the severity of the acidosis, relevant laboratory results, and the patient's age, weight, and clinical condition, as decided by a qualified doctor.
  3. Rapid injection of hypertonic solutions into neonates and children under two years of age should be avoided or the solution diluted to 4.2% to mitigate risks such as hypernatremia and intracranial hemorrhage.

 

Storage and Disposal

  • Store the injection below 30°C.
  • Protect from freezing and direct sunlight.
  • Keep this medicine out of the reach of children.
  • Discard any unused portion immediately after use, as the product contains no antimicrobial agent and is intended for single use in one patient on one occasion only.

Precautions and Safety

  • Administer the injection slowly to prevent fluid overload, particularly in patients with congestive heart failure, severe renal insufficiency, or conditions involving sodium retention.
  • Regularly monitor serum electrolyte levels (sodium, potassium, calcium) and blood gases to minimize the possibility of overdosage and metabolic alkalosis.
  • Avoid extravasation (leakage from the vein) during administration, as the hypertonic solution is irritant to veins and can cause chemical cellulitis, tissue necrosis, ulceration, or sloughing at the infiltration site.
  • Sodium bicarbonate is incompatible with certain substances in solution; consult specialized literature or a pharmacist before mixing with other drugs, especially calcium-containing solutions, to avoid inactivation or precipitation.
  • Ensure adequate alveolar ventilation during administration, particularly in cases of cardiac arrest, as proper ventilation contributes to acidosis correction and helps remove carbon dioxide released by bicarbonate.

Side Effects and Risks

  • Overly aggressive therapy can lead to metabolic alkalosis, characterized by muscular twitchings, irritability, tetany, muscle cramps, or spasms.
  • Hypernatremia (high sodium levels) may occur with excessive use.
  • Rapid administration can cause fluid overload, pulmonary edema, or swelling of the face, lips, tongue, or other body parts.
  • Redness, swelling, pain, or tissue damage at the injection site may occur if the solution leaks from the vein.
  • Other possible side effects include shortness of breath, wheezing, difficulty breathing, hypokalemia (low potassium levels), hypocalcemia, nausea, vomiting, or increased thirst.
