The Libetor 4ml Injection contains:

  • Labetalol Hydrochloride USP 5 mg/ml (active ingredient).
  • Anhydrous Dextrose 45 mg/ml.
  • Edetate Disodium 0.1 mg/ml.
  • Methylparaben 0.8 mg/ml (preservative).
  • Propylparaben 0.1 mg/ml (preservative).
  • Citric Acid Anhydrous and/or Sodium Hydroxide (to adjust pH).
  • Water for Injection.

Dosage and Administration

Libetor 4ml Injection is for intravenous use in hospitalized patients, with dosage individualized by the prescribing physician. It can be administered via repeated intravenous injection or slow continuous intravenous infusion.

  1. Repeated Intravenous Injection: An initial dose of 20 mg (4ml) is given by slow intravenous injection over 2 minutes. Additional injections of 40 mg or 80 mg can be administered at 10-minute intervals until the desired blood pressure reduction is achieved, up to a maximum cumulative dose of 300 mg.
  2. Slow Continuous Infusion: The injection can be diluted with compatible intravenous fluids to a concentration of 1 mg/ml or 2 mg/3 ml. The infusion is typically initiated at a rate of 2 mg/minute and adjusted according to the patient's blood pressure response.
  3. Patient Position: Patients should remain in a supine position during intravenous administration.

 

Storage and Disposal

  • Store Libetor 4ml Injection in its original package at room temperature, below 25°C (77°F), and protect it from light.
  • Do not freeze the injection.
  • If using a multi-dose vial, discard any unused portion 28 days after the initial puncture.
  • Dispose of any unused medication or waste material according to local guidelines and regulations.

Precautions and Safety

  • Dosage must be individualized, and patients should be carefully monitored for blood pressure and heart rate during and after administration.
  • Do not discontinue Labetalol HCl injection abruptly, especially in patients with heart disease, as this may exacerbate angina, lead to a heart attack, or cause abnormal heart rhythms.
  • Exercise caution in patients with liver impairment, as the drug is metabolized more slowly in these individuals.
  • Labetalol may mask symptoms of hypoglycemia in diabetic patients and alter glucose levels; monitor blood sugar frequently.
  • This medicine is contraindicated in patients with bronchial asthma, overt cardiac failure, greater than first-degree heart block, cardiogenic shock, or severe bradycardia.
  • Symptomatic postural hypotension (dizziness or lightheadedness upon standing) is a common side effect, particularly within 3 hours of administration.

 

Side Effects and Risks

  • Common side effects include dizziness, fatigue, nausea, lightheadedness, and increased sweating.
  • Report serious side effects to your care team immediately, such as allergic reactions (skin rash, itching, swelling), signs of heart failure (shortness of breath, swelling, sudden weight gain), liver injury (upper belly pain, yellowing skin/eyes, dark urine), or a slow heartbeat.
