Johnlee Calcium Gluconate 10ml Injection contains Calcium Gluconate 10% w/v and Calcium D-Saccharate 0.475% w/v, used to treat acute symptomatic hypocalcemia and manage cardiac toxicity in certain electrolyte imbalances.

Dosage and Administration

  1. This medicine is for intravenous (IV) use only. It must not be administered intramuscularly or subcutaneously, as this can lead to severe tissue necrosis and sloughing.
  2. The exact dose and administration rate must be determined by a qualified doctor, based on the patient's individual requirements, the severity of symptoms, and serum calcium levels.
  3. Dilute the injection prior to use in 5% dextrose or normal saline to achieve a concentration of 10-50 mg/mL.
  4. Administer the diluted solution slowly. The infusion rate should not exceed 200 mg/minute in adults and 100 mg/minute in pediatric patients.
  5. Patients should be monitored with ECG, blood pressure, and serum calcium level checks during administration.

 

Storage and Disposal

  • Store the injection below 25°C. Protect it from direct light and moisture.
  • Visually inspect the solution before administration; it should appear clear and colorless to slightly yellow. Do not use if any particulate matter or discoloration is observed.
  • Use the diluted solution immediately after preparation. Discard any unused portion appropriately.
  • Keep this medicine out of the reach of children.

Precautions and Safety

  • This injection is contraindicated in patients with hypercalcemia (high blood calcium), ventricular fibrillation, or severe renal failure.
  • Concurrent use with ceftriaxone is contraindicated in neonates (28 days of age or younger) due to the risk of fatal precipitates.
  • Exercise caution when administering to patients receiving cardiac glycosides, as synergistic effects can lead to cardiac arrhythmias. If co-administration is necessary, give slowly with close ECG monitoring.
  • Rapid intravenous administration can cause adverse reactions such as vasodilation, decreased blood pressure, bradycardia, cardiac arrhythmias, syncope, and cardiac arrest.
  • Avoid intramuscular or subcutaneous injection due to the high risk of severe tissue necrosis and sloughing.
  • This product contains aluminum, which may accumulate to toxic levels with prolonged parenteral administration, particularly in patients with impaired kidney function or premature neonates.
  • Calcium Gluconate Injection is incompatible with solutions containing phosphate or bicarbonate; mixing these may result in precipitation.

 

Side Effects and Risks

  • Common side effects include injection site reactions such as pain, swelling, and redness.
  • Rapid injection may lead to vasodilation, a drop in blood pressure, slow heart rate (bradycardia), irregular heartbeats, fainting (syncope), or cardiac arrest.
  • Symptoms of high calcium levels (hypercalcemia) may include increased thirst or urination, nausea, vomiting, confusion, unusual weakness or fatigue, and bone pain.
  • Allergic reactions, though rare, can manifest as skin rash, itching, hives, or swelling of the face, lips, tongue, or throat.
  • Other reported side effects include a change in taste and flushing, primarily over the face, neck, and chest, during injection.
  • Extravasation (leakage outside the vein) can cause tissue necrosis and calcinosis cutis.
