Dopactive 5ml Injection contains Dopamine Hydrochloride 200mg/5ml, a sympathomimetic agent used to treat low blood pressure and shock, and to improve heart function in certain critical conditions.

 

Benefits

Dopamine HCl helps normalise decreased blood pressure by increasing the heart's pumping capacity and improving blood flow to vital organs. It can also increase urine output and support heart function in cases of heart failure.

 

Uses and Indications

  • Low blood pressure (hypotension) due to various causes such as heart attack, severe infection (sepsis), or trauma.
  • Shock syndrome resulting from myocardial infarction, trauma, endotoxic septicemia, open-heart surgery, or renal failure.
  • Chronic cardiac decompensation, such as congestive heart failure.

The Dopactive 5ml Injection contains:

  • Dopamine Hydrochloride IP 200 mg (active ingredient) per 5 ml.
  • Excipients: Sodium metabisulfite, citric acid (anhydrous), and sodium citrate dihydrate.

Dosage and Administration

  1. Dopamine HCl Injection must be diluted before intravenous administration.
  2. It is administered as a continuous intravenous infusion, preferably into a large vein, using an infusion pump in an intensive care setting.
  3. The initial dosage for adults and pediatric patients typically ranges from 2 to 5 mcg/kg/minute, adjusted gradually in increments of 5 to 10 mcg/kg/minute based on the patient's response, up to a maximum of 50 mcg/kg/minute.
  4. Hypovolemia, acidosis, and hypoxia should be corrected before initiating Dopamine HCl Injection.
  5. Do not add additional medications to the diluted infusion solution.

 

Storage and Disposal

  • Store at 20° to 25°C (68° to 77°F); excursions permitted to 15° to 30°C (59° to 86°F).
  • Protect from freezing.
  • Do not use the injection if it is darker than slightly yellow or discolored in any other way.
  • Keep out of reach of children.

Precautions and Safety

  • Careful monitoring of blood pressure, urine flow, and cardiac output is necessary during infusion.
  • Dopamine is contraindicated in patients with pheochromocytoma or uncorrected tachyarrhythmias/ventricular fibrillation.
  • Tissue ischemia and extravasation can occur; infuse into a large vein and monitor the infusion site frequently.
  • Patients with a history of occlusive vascular disease should be monitored for changes in skin color or temperature of extremities.
  • Abrupt discontinuation can lead to marked hypotension; gradually reduce the infusion rate.
  • Contains sodium metabisulfite, which may cause allergic reactions in susceptible individuals.
  • Use with caution in patients with diabetes mellitus, glaucoma, or benign prostatic hyperplasia.

 

Side Effects and Risks

  • Common side effects include irregular heartbeats (tachycardia, palpitations, ectopic beats), changes in blood pressure (hypertension, hypotension), nausea, vomiting, headache, anxiety, chills, goosebumps, shortness of breath, and tremors.
  • Serious side effects can include severe peripheral and visceral vasoconstriction, cardiac arrhythmias, tissue necrosis from extravasation, and allergic reactions.
