• Bupivacaine Hydrochloride 5 mg/mL (0.5% w/v) (active ingredient).
• Dextrose Monohydrate 80 mg/mL (8% w/v) (baricity agent).
• Excipients: Water for injections, sodium hydroxide and/or hydrochloric acid (for pH adjustment), nitrogen.

Dosage and Administration

1. Bupixate Heavy 4ml Injection is administered as an injection into the lower part of the spine (intrathecal or subarachnoid space).
2. The dose is determined by the clinician based on the specific procedure, the patient's physical condition, and the desired duration and extent of anesthesia.
3. For spinal anesthesia in adults, typical doses range from 2-4 ml (10-20 mg Bupivacaine hydrochloride). The lowest effective dose should always be used.
4. The injection should be administered slowly, with aspiration performed before and during administration to prevent accidental intravascular injection.
5. This medicine is not for intravenous administration.
6. Dose adjustments may be necessary for elderly or debilitated patients, and those in the late stages of pregnancy.

Storage and Disposal

• Store the injection below 25°C (or 30°C), protected from direct sunlight and moisture.
• Do not freeze the solution.
• Keep this medicine out of the sight and reach of children.
• Once the ampoule is opened, the medicine must be used immediately.
• Discard any unused portion after administration.
• Do not use the solution if it appears discolored or contains any particulate matter.

Precautions and Safety

• Bupixate Heavy 4ml Injection should only be administered by clinicians experienced in regional anesthesia techniques.
• Resuscitative equipment and emergency drugs must be immediately available during administration.
• Constant monitoring of cardiovascular and respiratory vital signs, along with the patient's state of consciousness, is crucial after injection.
• Avoid rapid injection to minimize the risk of significant hypotension and other hemodynamic changes.
• This medicine is contraindicated in patients with known hypersensitivity to bupivacaine, other amide-type local anesthetics, or any of its excipients.
• It should not be used in cases of severe shock, severe hypotension, severe hemorrhage, septicemia, or local infection at or near the injection site.
• Use with caution in patients with severe renal dysfunction, liver disease, heart problems, or those concurrently receiving other local anesthetics or anti-arrhythmic drugs, as toxic effects can be additive.
• Intravenous regional anesthesia (Bier Block) is not an approved use for this product.
• Avoid administration during uterine contractions, as this may lead to an undesirably high motor block.
• There is a risk of methemoglobinemia; monitor for signs such as cyanotic skin discoloration.
• Patients should not drive or operate machinery on the day of surgery due to potential effects on reaction ability and muscle coordination.
• Alcohol consumption should be avoided during treatment, as it may lead to a drop in blood pressure.

Side Effects and Risks

• Common side effects include nausea, vomiting, dizziness, blurred vision, drowsiness, ringing in the ears, numbness, feeling disoriented, headache, backache, slow heart rate, low blood pressure (hypotension), paresthesia (tingling or pricking sensation), and injection site reactions.
• More serious, though less common, side effects can include extensive or total spinal blockade (potentially leading to cardiovascular and respiratory depression, profound hypotension, bradycardia, or cardiac arrest), convulsions, and allergic reactions (e.g., skin rash, difficulty breathing, swelling).
• Other possible effects reported are taste changes, decreased sensitivity to stimuli (hypoesthesia), anemia, increased C-reactive protein, fever, procedural pain, discomfort when urinating, anxiety, stiff muscles, trembling, difficulty breathing, and low body temperature.
