Amidone 150mg Injection 3ml contains Amiodarone Hydrochloride, a potent antiarrhythmic agent used to manage and prevent life-threatening irregular heartbeats.

The Amidone 150mg Injection 3ml contains:

• Amiodarone Hydrochloride I.P. 150 mg/3ml (equivalent to 50 mg/mL) (active ingredient).
• Benzyl Alcohol 2%w/v (as preservative).
• Other non-medicinal ingredients may include polysorbate-80, sodium hydroxide and/or hydrochloric acid, and water for injection.

Dosage and Administration

1. Amidone 150mg Injection 3ml is administered intravenously, either as an infusion or a slow injection, strictly under hospital or specialist supervision.
2. The initial loading dose is typically 5 mg/kg bodyweight, diluted in 250ml of 5% dextrose solution, and infused over a period of 20 minutes to 2 hours. Subsequent maintenance infusions will be adjusted based on the patient's clinical response.
3. In extreme clinical emergencies, such as cardiac arrest due to defibrillation-resistant ventricular fibrillation, a bolus injection of 300mg (or 5mg/kg) diluted in 20ml of 5% dextrose may be administered rapidly.
4. This injection must only be diluted in 5% Dextrose (Glucose) solution; it is incompatible with 0.9% Sodium Chloride solution.
5. Do not mix other medicinal preparations in the same syringe.
6. For repeated or continuous infusions, administration via a central venous route is recommended to minimise the risk of local reactions like phlebitis.

Storage and Disposal

• Store the injection between 15°C and 30°C (68°F to 77°F).
• Protect from light and excessive heat; keep the ampoule in its original carton until immediately before use.
• Keep this medicine out of the reach of children.
• This is a single-use vial; any unused portion must be discarded.

Precautions and Safety

• Amidone 150mg Injection 3ml should only be used in facilities equipped for cardiac monitoring, defibrillation, and cardiac pacing.
• Do not use this medicine if you have a known hypersensitivity to amiodarone, iodine, or any of its components.
• It is contraindicated in patients with severe heart conditions such as marked sinus bradycardia, second- or third-degree atrioventricular (AV) block (unless a functioning pacemaker is present), cardiogenic shock, or severe hypotension.
• Exercise caution in patients with pre-existing liver disease, thyroid problems, or lung disorders, as amiodarone can worsen these conditions.
• This medication may cause photosensitivity; avoid direct sunlight exposure and use protective clothing.
• Due to the presence of benzyl alcohol, this injection is contraindicated in neonates, infants, and children up to 3 years old.
• Pregnant or breastfeeding women should consult their doctor before use; this medicine is generally contraindicated during pregnancy and breastfeeding unless deemed life-threatening.
• Avoid consuming grapefruit juice while on this medication.
• Always inform your anaesthetist that you are receiving amiodarone before any surgical procedure.

Side Effects and Risks

• Common side effects include nausea, vomiting, constipation, changes in taste, slow heart rate, decreased blood pressure, and reactions at the injection site (pain, swelling, redness).
• Serious side effects can include liver problems (jaundice, fatigue, loss of appetite, stomach pain), lung inflammation (shortness of breath, persistent cough, wheezing), worsening or new irregular heartbeats, very slow heartbeats, and severe allergic reactions (rash, swelling of face/throat, difficulty breathing).
• Other potential risks include thyroid dysfunction (hyper- or hypothyroidism), vision problems, and a blue-grey discoloration of the skin.
